Indiana Care Circle is a supportive network of professionals who meet monthly to exchange ideas and problem solve for the benefit of our clients.
We share with one another first-hand knowledge about subjects such as changes in legislation, program funding, local and national resources, current events, and any other information that could be helpful in serving a client in need.
Our ongoing professional relationships provide a community of trust and goodwill among professionals who do the best they can to serve each and every client.
Membership is open to individuals or organizations involved in, or associated with, the continuity of care process, and who are committed to the purpose of Indiana Care Circle.
Statement of Purpose: Indiana Care Circle (ICC) exists to enhance continuity of care as an essential component of the health care delivery system and an integral part of the total patient care. ICC accepts the responsibility of promoting the development and systematic implementation of the highest standards of continuity of care. ICC serves as a medium for the sharing of professional problems and solutions, and resources in the development of health care programs. ICC also serves to promote administrative awareness of the standards, structure, and organization of a continuity of care program.
Continuity of Care: The World Health Organization defines Continuum of Care as "the provision of comprehensive care from the hospital to the home, which advocates the pooling together of medical and social services within the community and the creation of linkages between community care initiatives at all levels of the health care system." Indiana Care Circle is dedicated to continual improvement of the ways this concept is working in the Tippecanoe County area.
